1.  Download membership form (from below or right hand block)

2.  Send filled Membership form along with the DD/Cheque of specified amount according to your membership option to the registered office address.
    Cheque/DD should be drawn in favour of St. John's Marhauli Alumni Association payable at Varanasi
(please mention  your name, batch and permanent address at back od DD/Cheque)

Membership Fee :
Annual Member: Rs 400/-
Life Member: Rs 3000/-

Instructions for filling of FORM:
1. Name : name of alumni
2. Pass Out Batch: Year of batch in which he passed class 12th, the batch must have passed out two years before i.e. batch 2005 and before only.
3.Date of Birth: DOB of Alumni
4.Permanent Address: permenant Address of alumni
5. Present Address: Correspondance address
6. Tel, Mobile,E-mail: Telephone number, mobile no., E-mail of Alumni
7.Present Occupation: occupation of alumni
8. Hobbies: Interest's and hobbies of alumni
9.Married: Yes/No IF YES, then details of spouse like Name, DOB, and  Wedding Anniversary
10. Year of Admission:in paarticular class
11. Year of Pass out: from school, it may be before 12th also and Class
12.Membership Detail: mark on appropiate membership option and make payments accordingly. Write ypur details on back of DD/Cheque.
